I, applicant, understand that this is not a paid position. I agree that all the work I do is on a volunteer basis and I am not eligible to receive any monetary payment or reward. I will not hold Million Little or its employees, board, associates, and/or clients responsible for any injuries or losses, both corporal and/or property-related, that may occur during my volunteer hours. I understand that because I am not an employee of Million Little Worker's Compensation is not provided by the organization.
I understand that in the course of considering my application, Million Little may inquire to verify my information, including my background. I understand that photographs or videos may be taken of my volunteer activities. I authorize Million Little, without limitation, to copy, publish, exhibit, or distribute such photographs, videos, and/or documentation for the purpose of reporting/promotion of volunteering and education. I waive all rights or claims I may have against Million Little and/or its agents, subsidiaries, or assignees related to the above photos, videos, and documentation.
I understand that all reports, studies, information, data, statistics, forms, designs, plans, procedures, systems, videos, images, and any other materials produced by myself during volunteer hours under supervision of Million Little shall be the property of Million Little.
I understand that by volunteering with Million Little that I will learn valuable information and techniques developed by Million Little and will have access to various sites and programs that retain Million Little. I agree that I will not use such information or access to compete with Million Little and will not offer similar services to such sites or programs or which may affect Million Little's relationship with such sites or programs.
As a volunteer, I agree to abide by the policies and procedures of Million Little.

I, the parent/legal guardian of the applicant, have reviewed above statement and agree. I will not hold Million Little, its employees, associates and/or clients responsible for injuries or losses, both corporal and property-related, that may occur during the time that my child is volunteering. I give my child permission to volunteer at Million Little.
